Re-schedule Request Form

Please read through the following terms and conditions for Re-scheduling request and select all the boxes below to proceed with the request:

I understand that I will need to request for a re-schedule if I am unable to meet the 75% mandatory attendance requirement. Otherwise, I will be deemed to have voluntarily withdrawn from the programme and IAL reserves the right to reclaim the funded amount that was accorded to me upon enrolment. Subsequently, I will not be eligible to receive funding for the same programme/module.
I am expected to carry out self-directed learning if my absence from a training session DOES NOT affect the minimum attendance of 75% or the practical performance components as no replacement classes or additional coaching will be provided.
I understand that re-scheduling is only free-of-charge for Recognised absences and supporting documents need to be provided.
I understand that a non-refundable admin fee of $119.90 (inclusive of 9% GST) is applicable for request to re-schedule beyond the allowable Recognised Absences.
My request will be rejected if I submit any inaccurate or incomplete information.
